Early detection of circulatory disorders is key to preventing serious vascular conditions such as peripheral artery disease (PAD), blood clots, and arterial blockages. The Ankle-Brachial Index (ABI) test is a non-invasive vascular screening tool used to measure blood flow in the legs and assess circulatory health.

At Vascular Surgery Associates in Los Angeles & Santa Monica, we provide accurate ABI testing to help diagnose vascular conditions early, ensuring patients receive the most effective treatments before complications arise.

What Is an Ankle-Brachial Index (ABI) Test?

The ABI test compares blood pressure readings in the ankles and arms to evaluate how well blood is flowing to the lower extremities. A significant difference in pressure may indicate blockages or narrowing of the arteries, which are signs of PAD or other vascular diseases.

The test is:

  • Quick and painless, taking less than 15 minutes
  • Non-invasive, requiring only a blood pressure cuff and Doppler device
  • Highly effective in diagnosing circulatory issues before symptoms worsen

Why Is ABI Testing Important?

According to the American Heart Association, nearly 8.5 million people in the U.S. have PAD, yet many cases go undiagnosed. Early detection with ABI testing allows for:

  • Early intervention to prevent complications such as ulcers, gangrene, or amputation
  • Better management of vascular conditions, including high blood pressure and diabetes
  • Improved treatment planning for patients with leg pain, swelling, or numbness

Who Should Get an ABI Test for Vascular Screening?

ABI testing is recommended for individuals who:

  • Are over 50 and have risk factors such as diabetes or smoking history
  • Experience leg pain, cramping, or numbness while walking
  • Have a history of heart disease, high cholesterol, or high blood pressure
  • Have slow-healing wounds or ulcers on their legs or feet
  • Show signs of poor circulation, including cold feet or skin discoloration

How the ABI Test Works

During the Ankle-Brachial Index test, a vascular specialist will:

  1. Measure blood pressure in both arms and ankles using a blood pressure cuff and Doppler ultrasound device
  2. Compare the readings to calculate the ABI ratio
  3. Interpret results to determine if narrowed or blocked arteries are affecting circulation

A normal ABI ratio is between 0.9 and 1.3. Lower values may indicate PAD, blood clots, or arterial narrowing, while higher values could signal vessel stiffening from diabetes or other conditions.

What Happens After an ABI Test?

If an abnormal ABI result suggests circulatory issues, further testing may be recommended, including:

  • Vascular Ultrasound for detailed imaging of blood flow
  • CT or MR Angiography to identify blockages in the arteries
  • Additional vascular screenings for patients at high risk of heart disease or stroke

Learn How Our Expert Care Has Changed Lives for the Better

Dr Tulloch and his very competent and experienced team performed three relatively minor procedures to improve the circulation in my lower legs. ( I am 80.) After only six weeks, improvements were very noticeable, including reduced swelling, shrinking of three small venous ulcers, improved skin condition and easier walking / hiking.
